Signature Forms

1. The Signature Forms MUST be: Generated from Visual and NOT hand-written. The name of the insured, as shown on the assignment and then the word Signature. NOTHING MORE, NOTHING LESS. If the auditor does not do this, the reviewer must correct. Mad House Café Signature OR Brown, Joe Signature. Same with Time Cards: Brown, Joe Time Card.
